私隱政策聲明 Privacy Policy Statement

尚健國際有限公司 (Celki International Limited) (並以尚健醫療器材公司 (Celki Medical Company)（下稱「公司」、「我們」）營運) 尊重閣下的私隱權利。我們遵守香港特別行政區法例第 486 章《個人資料(私隱)條例》(下稱「條例」) 執行私隱政策。本聲明旨在通知您所收集個人資料的目的、收集方式、存儲和使用方法，本公司如何保護個人資料，以及應向何人提出查閱資料的要求。

Celki International Limited trading as Celki Medical Company (“Company”, “we”) respects your rights to privacy.  This Statement has been developed in line with the requirements of the Personal Data (Privacy) Ordinance (Cap 486) of the Hong Kong Special Administrative Region (the “Ordinance”).  This Statement is intended to keep you informed as to why personal data is collected, how it is collected, stored and used, how the Company protects personal data and to whom the Company would allow data access requests. 

 

1. 收集個人資料的目的和方式 Purpose and Manner of Collection

本公司將通過合法和適當的方式收集個人資料。您理解並認同，您自願提供個人資料，惟若您無法提供相關資料, 或提供的資料不正確或不齊全，我們可能無法為您提供服務。The Company will collect personal data by lawful and appropriate means. You understand and acknowledge that provision of personal data by you is voluntary.  A failure to provide the requested personal data, or the provision of inaccurate or incomplete information may result in the Company not being able to provide you the requested service from the Company.

 

本公司可能使用您的個人資料作包括但不限於以下目的：Your personal data collected may be used by the Company for the purposes of, including but not limited to,:

i） 處理您的查詢及/或相關服務；handling your enquiries and/or related services;

ii） 就診斷服務及/或相關服務進行調查/問卷調查；conducting surveys/questionnaires for diagnostics services and/or related services;

iii）註冊我們的 celkicare.com 以監控家居醫療處方，並跟蹤本公司產品和服務的表現；registering for an account on celkicare.com to monitor the homecare therapy prescribed and to keep track of the performance of the products and the services of the Company; 

iv) 發放醫療設備出廠後安全通知及處理相關行動；releasing field safety notices and handling related actions.

v）處理本公司的網上購貨交易； handling online purchase transactions from the Company's website;

vi） 本公司處理您的求職申請； processing your job application with the Company; or

vii) 作直接促銷用途。 direct marketing.
 

2. 準確性和保存期 Accuracy and Retention Period

本公司旨在讓您的個人資料在記錄中保持準確和最新, 我們不會保存超過上述使用用途的所需的時間。

The Company aims to keep your personal information on its records accurate and up-to-date. We will not keep your personal information for a duration longer than necessary for any of the aforementioned purposes.  

 

3. 資料的使用 Use of Information

除了本聲明條款 1所述之目的,  本公司誠意向您提供最新的資訊以及產品/服務信息，例如研討會、活動、折扣、優惠和任何推介活動。此等信息可透過您的電子郵件地址，通訊地址及/或電話號碼傳送給您。若您欲停止或開始接收此等信息，您可以將請求發送至 celki.cs@airliquide.com，並說明您的註冊名稱和聯絡資料。

In addition to the purposes set out in Clause 1 of this Statement, it is the Company’s intention to provide you with the latest news and products/services information, such as seminars, activities, discounts, benefits and any promotions. Such information may be delivered to you to your email address, correspondence address and/or telephone number. If you want to stop or start receiving such information, you can send your request to celki.cs@airliquide.com with your registered name and contact information.

 

4. 信息傳遞 Transfer of Information

對本聲明條款 1 所述之目的，本公司可於香港或香港以外的地方使用您的個人資料 (包括醫療數據) 或向以下各方提供此等資料：-

For purposes set out in Clause 1 of this Statement, the Company may provide your personal data (including medical data) to the following parties, which can be within or outside Hong Kong:-

 

i) 向本公司提供與其業務運作有關服務的任何代理，承辦商或第三方服務供應商，包括行政、電訊、電腦、數據處理儲存以及本公司提供服務時附帶的其他服務（例如專業顧問或香港以外其他司法管轄區的服務代理，該司法管轄區可能有, 也可能沒有就類似的條例立法以保護個人資料)；any agent, contractor or third party service provider who provides services to the Company in connection with the operation of the Company's business including administrative, telecommunications, computer, data processing storage as well as other services incidental to the services offered by the Company (such as professional advisors or providers in another jurisdiction outside Hong Kong which may or may not have legislations similar to the Ordinance to protect personal data);

ii) 對本公司負有保密責任的任何個人或實體，承諾對該等資料保密；any person or entity under a duty of confidentiality to the Company which has undertaken to keep such personal data confidential;

iii) 根據監管機構或其他機構發布的對公司有約束力或預期公司將遵守的任何法律或法規、規章、業務守則或準則的要求，本公司有義務向其披露信息的任何個人或實體；any person or entity to whom the Company is under an obligation to make disclosure under requirements of any law or rules, regulations, codes of practice or guidelines issued by regulatory or other authorities binding on the Company or with which the Company is expected to comply;

iv) 與本公司的權利或業務相關的任何實際或擬議的受讓方，承讓人，參與者或分參與者； any actual or proposed assignee, transferee, participant or sub-participant of the Company’s right or business;

v) 本公司及/或本公司集團內公司的任何人士或部門； any person or division within the Company and/or the Company group of companies;

vii) 本公司或產品製造商下的各類醫療數據儲存平台或其他第三方機構，包括但不限於承辦商、任何電訊營運商、任何第三者收賬機構、任何信用諮詢機構、任何保安機構、任何信貸供應商、銀行、金融機構、向本公司負有保密責任的我們任何實際或建議受讓人; 及/或 medical data storing platforms under the Company or product manufacturers, or to other third-party organizations, including but not limited to contractors, any telecommunications providers, any third party payment collection agencies, any credit reference agencies, any security agencies, any credit providers, banks, financial institutions, any of our actual or proposed transferees who owe the Company a duty of confidentiality; and/or

viii) 本公司職員及其他醫生及醫護人員透過本公司或產品製造商下的各類醫療數據儲存平台查閱顧客的個人資料及醫療數據。數據之使用權及目的為讓本公司或醫護人員作顧客治療及相關的跟進服務。以保持服務質素及診斷的準確性。Staff of the Company and other consulting medical professionals who have access to such information on all kinds of medical data storing platforms under the Company or product manufacturers for treatment and related follow-up services to maintain the quality of service and accuracy of any diagnosis.

 

5. 個人資料的保安 Security of Personal Data

本公司致力確保您的個人資料免受未經授權或意外查閱、處理或刪除。本公司將實施適當的實質、電子及管理措施，以保護和保障您的個人資料。

任何與本網站鏈接的網站均可能收集您的資料。在您向他們提供資料之前，請務必細閱相關的私隱聲明。您向任何第三方提供的資料與本公司無關，本公司對此不承擔任何責任。

The Company will strive at all times to ensure that your personal data will be protected against unauthorized or accidental access, processing or erasure. The Company will implement appropriate physical, electronic and managerial measures to safeguard and secure your personal data.

Any website with links to this Website may collect your data. Please be advised to read relevant privacy statements carefully before you provide your data to them. Your provision of your data to any third party has nothing to do with the Company and the Company will not have any responsibility on such.

 

6. 查閱個人資料 Access to Personal Data

根據該條例，您有權：Subject to the provisions in the Ordinance, you are entitled-

i) 查閱由您提供給本公司的個人資料； to request access to personal data provided by you;

ii) 要求本公司更正由你提供的個人資料；to request the Company to correct any personal data provided by you;

iii) 確定本公司有關個人資料的政策及實務，並知悉本公司所持有由您提供個人資料的種類； to ascertain the Company’s policies and practices in relation to personal data and to be informed of the kind of personal data provided by you and held by the Company;

iv) 要求本公司停止使用您的資料作直接促銷用途。 to request the Company not to process personal data provided by you for direct marketing purposes.

根據該條例，本公司有權就處理您的資料查閱服務要求收取合理費用。

According to the Ordinance, the Company has the right to charge a reasonable fee for handling your data access request.
